Job Summary: We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team, providing essential medical evaluation, treatment, consultation, and education to clients in a non-institutional setting. This role is intended for outpatient environments, such as county health departments, clinics, schools, and other community settings. The LPN will work under the supervision of an advanced practice registered nurse, registered nurse, licensed physician, or other authorized practitioner as defined by South Carolina law.
Responsibilities:
- Conduct health screenings and collect relevant health information.
- Provide client education on nutrition, health promotion, and disease prevention.
- Maintain detailed and accurate medical records.
- Communicate effectively with clients, families, and healthcare team members, both orally and in writing.
- Comply with federal and state regulations and laws related to healthcare practices.
Knowledge, Skills, and Abilities:
- Proficient in collecting and documenting health screening information.
- Ability to educate clients on health and wellness topics, including nutrition and disease prevention.
- Strong attention to detail and accuracy in medical documentation.
- Effective communication skills, both oral and written.
- Familiarity with federal and state regulations related to nursing practice.
Qualifications:
- Licensure: Active South Carolina LPN license.
- Certifications: Basic Life Support (BLS) with CPR and AED; certification in nutrition or health promotion (preferred).
- Completion of an approved nursing program per SC Nurse Licensure Act, Section 40-33-32.
- Valid driver’s license (may be required).
Qualifications:
- High school diploma or GED.
- Certification as a Medical Assistant, Certified Nursing Assistant (CNA), or Behavioral Health Technician (BHA).
- Prior experience in a healthcare or patient-care setting preferred.
Special Requirements:
- This role requires the LPN to work in outpatient settings, potentially involving travel to various community sites.
Work Environment:
- Hands-on role, which may involve standing for long periods, lifting patients, and assisting with patient mobility.
